Community Referrals by EMS (CREMS)

From 2008 to 2020 inclusive, this dataset provides the number of referrals made by frontline Paramedics who identify patients whom they feel should be assessed by Local Health Integration Network (LHIN) Home and Community Care teams to confirm whether these patients require alternative or additional healthcare options. These referrals are made through the electronic patient care record and securely received by the LHIN
